On behalf of St. John the Baptist Catholic Church, we extend to you and your child our warmest welcome. Baptism is a welcoming of your child into Christ and into the Church. A copy of your child’s birth certificate must accompany this form.

Requirements:
- the original Birth certificate for the child or adult. A copy will be made here and the original returned to the parent or adult requestor.
- a photo ID of the parent or adult requestor. A copy will be made here at the time of request.
- the same individual and photo ID presented at the time of pick-up.
Certificates for adults will only be released to the adult on the certificate or the church requesting the certificate.

Please be advised, fraudulent documents will be reported to the appropriate authorities.

(An individual must be baptized at St. John’s for a certificate to be prepared. We do not prepare baptism certificates for other parishes)
